Access
All patient notes are treated with the strictest confidence and we comply with the Data Protection Act 1998. Those authorised to access medical records are:
- All practice staff, along with attached professionals. They may access information to enable them to carry out their duties. Occasionally, other health professionals such as the health visitor, midwife, district nurse and community psychiatric nurse and other organisations or teams involved in your management may ask for specific information about your health.
- Third parties, eg a family member or solicitor, when the practice has been given your consent in writing.
When giving your consent to supply data to third parties acting on your behalf, we request that you specify whether you wish all your medical records or only those relating to a specific event or period to be submitted.
